1. Call any of the participating dentists on the back of the certificate and set your appointment.
2. Go to your initial appointment where the doctor will do a Basic Dental Exam and take Custom Impressions of your upper and lower teeth.
3. When the dental office calls, go back and pick up your Custom Whitening Trays and Gel.
4. Wear you trays 15- 30 minutes a night for 1-2 weeks or as directed.
